4-H HEALTHY CHOICES (Summer 2021) - PRE-REGISTRATION *  Brewster, NY

Monday, July 19 - Thursday, July 22, 2021  *  9:30am - 11:30am 

Open to Putnam County Youth ages 9 & up; First Paid / First Serve. (Non-Residents may be considered if the program does not fill up).    

This 4-H Programs meets NYS Ed Learning Standards for Personal Health & Fitness. 

PLEASE NOTE: YOUR PRE-REGISTRATION IS NOT COMPLETE UNTIL PAYMENT and ALL required 4-H Enrollment / Membership Forms HAVE BEEN RECEIVED. Full payment must be received within 5 business days or your child's registration will be canceled. If you decide to pay with PayPal, please make sure after submitting the registration form that on the next page you click on "add to PayPal cart" to continue with your purchase. 

PLEASE Have Your Child:

• Bring a reusable water bottle (daily)


• Wear sneakers & comfortable clothing


• A Mask (Only for indoor settings/Social-Distancing will remain in effect)


• PARENTS: Please pick up your child promptly at 11:30am

The Registration Fee includes all supplies & materials.
