2023 Putnam County Youth Forum - Workshop Selection Form:  ATTENTION: Before making your Workshop Selections, YOU MUST submit a completed Parental Permission Slip to your School Liaison. (PLEASE NOTE: A School email will NOT work with this Registration system). If you do not have a Student Registration Packet / Permission Slip, please see:

The Putnam County Youth Forum is hosted by Cornell Cooperative Extension of Putnam County's 4-H Youth Development Program, the Putnam County Youth Bureau, NYS OCFS, & Camp Herrlich. 

This educational field trip is supported by Brewster, Carmel, Haldane, Mahopac, and Putnam Valley High Schools.  A hot lunch is provided during the event and transportation to and from the Youth Forum is provided by your child's school. 

Please click on the REGISTER button below to select your Youth Forum workshop choices.  In order to complete your registration, you must also submit your Parental Permission slip to your school's liaison by or before April 14th, or your workshop selections will be deleted.   

We will do our best to accommodate your workshop selections, however due to time constraints, changes may be made to your workshop schedule.
