Family Preparedness Clinic - Cornell Farmworker Program

  • Power of Attorney
  • Parental Designation Forms
  • Parental Permission to Travel

Documents Needed:
The parents/guardians must present a valid ID.
Examples: Valid driver's license,
passport, government ID.
You must bring an original or
document for you and your child to prove
parent.
Examples: U.S. birth certificate, Foreign birth certificate.
If custody of the child was given to you thr
court, please bring the Court Order.
Important! Information for the person designated
We suggest bringing the person designated to attend with
you. If that is not possible, bring
person designated. You will need his/her name, physical
address, phone number and date of
person must reside in New York State and

designated in parental relationship must sign the document
in front of a notary. If designated person is attending to
sign, he/she should bring an ID.
